Abstract

대구시에 소재한 일개 행정구의 중학생 1,156명(남학생 570명, 여학생 586명), 고등학생 1,001명9남학생 581명, 여학생 420j명) 등 2,157명을 대상으로 1999년 4-5월 자기기입식 설문조사로 흡연 및 음주실태를 파악하고 관련요인을 분석하였다.
남자중학생의 흡연 경험률은 13.3%, 현재 흡연률은 2.4%였으며 여자중학생은 각각 8.1%, 1.8%였고, 남자고등학생 각각 33.9%, 12.7%, 여자고등학생 각각 10.3%, 2.9%였다. 음주 경험률과 현재 음주율은 남자중학생은 각각 210.9%, 3.35, 여자중학생 22.0%, 3.9%, 남자고등학생 60.%, 21.6%, 여자고등학생 56.8%, 11.2%였다.
중학생의 경우 중학생과 고등학생이 흡연과 음주를 하는 것이 바람직하지 못하다는 응답이 절반을 넘었으나, 고등학생의 경우 고등학생이 음주를 하는 것이 바람직하지 못하다는 응답은 남녀 각각 25.7%, 31.35에 불과하였다. 남자중학생의 80.7%, 여자중학생의 83.7%, 남녀 고등학생 각각 63.5%, 78.8%가 흡연과 음주 모두가 건강에 악영향을 미친다고 응답하였다.
초등학교 때의 흡연 경험률은 남자중학생의 6.3%였으며, 여자중학생 3.6%, 남자고등학생 5.2%, 여자고등학생 1.3%로 남녀 모두에서 고등학생에 비해 중학생의 초등학교 흡연 경험률이 높았으며, 초등학교 음주 경험율도 같은 경향이었다. 흡연 경험군과 음주경험군의 동기는 흡연과 음주 모두에서 호기심으로가 가장 높았다.
현재 흡연구의 일일 흡연량은 1일 반갑 미만이 대부분을 차지하였으며, 현재 음주군의 음주 빈도는 월 2-3회 이하가 가장 높았고, 1회 음주량은 90%이상이 반병 이하였다. 현재 흡연하고 있는 남녀 중·고등학생의 과거와 비교한 현재 흡연상태는 안하다가 다시 하고 있다가 가장 높았으며, 금연의향률은 남녀 중·고등학생 모두에서 60% 이상이었다. 현재 음주군의 음주상태는 남자 중·고등학생과 여자중학생은 `안하다가 다시 한다`라고 응답한 비율이 가장 높았고, 여자고등학생의 경우는 `이전과 비슷하다`라고 응답한 비율이 갖아 높았다. 금주 의향률은 중학생은 남·녀 각각 61.9%, 45.8%로 절반 정도를 차지하였으나 남·녀 각각 고등학생의 경우는 각각 30.2%, 17.1%로 낮았다.
남녀 중·고등학생 모두에서 담배구입의 주 경로는 담배가게였으며, 담배의 구입이 쉽다는 응답이 남녀 중·고등학생 모두에서 60%를 넘었다. 현재 흡연군의 부모 인지여부는 남녀 중·고등학생 모두 `부모 모두 모른다`가 가장 높았고, 학교에서 흡연하는 비율은 남자중학생 16.7%, 여자중학생 44.4%, 남자고등학생 55.7%, 여자고등학생 20.5%였다. 현재 음주군의 부모 인지여부에서는 여자중학생은 `부모 모두 모른다`가 가장 높았으나, 남자 중·고등학생과 여자고등학생은 `부모 모두 알고 있다`가 가장 많았다. 음주시 술집이 주 음주 장소인 비률은 남녀 중학생은 각각 16.7%, 20.8%였으며, 남녀 고등학생은 각각 54.1%, 47.95였다.
청소년의 흡연과 음주에 영향을 미치는 요인을 알아보기 위한 다변량 분석 결과, 주거지역, 부모의 음주정도, 학교성적, 흡연이나 음주하는 친구비율, 흡연과 음주의 건강 악영향에 대한 생각 등이 영향을 미치고 있었다.
따라서 청소년의 흡연과 음주에는 가정의 일반적 환경, 부모환경, 학교생활, 흡연과 음주의 건강 악영향에 대한 지식 등이 복합적으로 영향을 미치고 있으므로 청소년의 흡연과 음주의 예방을 위하여는 가정, 학교, 사회 등이 서로 유기적으로 협력하여 교육 지도해 나가야 될 것으로 생각되는데, 특히 어떤 친구와 사귀는지를 주의깊게 관심을 가지고 관찰해 보아야 할 것이며 흡연과 음주를 하는 또래집단에 대한 적극적인 계도와 교육이 요망된다.

To identify the smoking and alcohol abuse status and its related factors of middle and high school students in Taegu, the study was performed from April to May, 1999. The study population were selected by cluster sampling method and total 2,157 students replied to the self-administered questionnaire survey(1,156 middle school students, 1,001 high school students).
The major findings were as follows;
The experience rates of smoking were 13.39´o in male middle school students, 8.1% in female middle school students, 33.9% in male high school students, 10.3% in female high school students. It was higher in male than female students. The experience rates of alcohol abuse were 21.9% in male middle school students, 22.0% in female middle school students, 60.7% in male high school students, 56.8% in female high school students. There was no difference between male and female students.
Over half of middle school students replied that smoking and drinking of students was not desirable, but only a quater of high school students replied that drinking of high school students was not desirable.
The experience rates of smoking at primary school were 6.3% in male middle school
students, 3.6% in female middle. school students, 5.2% in male high school students, 1.3% in female high school students. It was higher in middle school students than high school students. The experience rates of alcohol abuse at primary school had a similar trend.
The most important motive of smoking and alcohol was `curiosity.´
Amount of smoking per day were below half pack, frequency of drinking was under 3 times per month and amount of drinking per a time were under a half bottle of Soju.
The main route of obtaining tobacco was tobacco store and the rate that the tavern was main place of drinking was 16.7% in male middle school students, 20.8% in female middle school students, 54.1% in male high school students, 47.9% in female high school students.
In multivariate analysis, smoking and alcohol abuse had significant association with general characteristics such as residency area, parents environment (parent´s smoking and drinking), school life (lower school grade, intimate friend´s smoking and alcohol abuse), knowledge for the health effect of smoking and alcohol abuse.
On consideration of above findings, to prevent students from smoking and drinking, we have to try together in house, school, and society.
